BACKGROUND
Site specific hazard awareness training is required for independent contractors (including commercial over-the-road truck drivers). Site specific training is not required for any person who is accompanied at all times by an experienced miner who is familiar with hazards specific to this mine. Site-specific information or instruction include hazards a person could be exposed to while at the mine, as well as applicable emergency procedures. These hazards could be unique geological or environmental conditions, recognition and avoidance of hazards such as electrical and powered haulage hazards, traffic patterns and control, restricted areas, warning and evacuation signals, evacuation and emergency procedures, or other special safety procedures.

Training can include written hazards, oral instruction, signs and posted warnings, walk-around training, or other appropriate means that alert persons to site-specific hazards at the mine.

Each independent contractor that mines (loads their own vehicle) at this location has the primary responsibility of providing new miner, newly-hired experienced miner, new task, and annual refresher training for their employees. R.W. MILLER & SONS, INC. will provide information to each independent contractor on site-specific hazards and the obligation of the contractor to comply with Part 46 regulations.

Independent contractors must inform the production-operator/ R.W. Miller office of any hazard they create in the mine.

COMPLIANCE

R.W. MILLER & SONS, INC. will identify site-specific hazards through posted signs and warnings and written handouts including maps of hazard sites and traffic patterns. Independent contractors agree to sign a form stating that they have had the required training applicable to MSHA Part 46 previous to entering the mine.

  1. Notify R.W. MILLER & SONS, INC. office.
  2. Depending on the emergency, take appropriate action:
  3. Severe Weather: Take shelter in the shop.
  4. Vehicle Accident: Dial 911 if needed. Take photos and get necessary insurance information.
  5. Pedestrian Accident: Dial 911. If you're medically trained, please provide assistance.
  6. Fire: Dial 911. Use extinguisher if it's a small fire, and if you're trained to use it.
  7. Flooding: Do not try to drive through standing water after a heavy rain. What looks like a puddle can be two feet deep.
  8. First aid kits are available in the Office/Scale House.

R.W. MILLER & SONS, INC.  PIT ACCESS AGREEMENT

All drivers must agree to abide by these Pit Access Rules to have access to R.W. MILLER & SONS, INC. Aggregates Operations:

1. Drivers should follow defensive driving techniques when in the pit.
2. Speed limit in production and loading areas is FIFTEEN miles per hour.
3. Drivers will obey all posted signs and warnings.
4. Drivers can stop at scale house to contact loader operators .
5. Drivers must be in their truck when it is being loaded.
6. Drivers are not permitted outside of their trucks in production or loading areas.
7. Hard hats will be worn when drivers are outside their truck.
8. All trucks must stay a minimum of two truck lengths away from any bank higher than their vehicle.
9. Drivers are allowed on designated roadways only (see map).
10. Drivers are not allowed in restricted areas (see map).
11. Drivers are required by Wisconsin State law to come to a complete stop before entering State Highway 120.
12. Drivers must be accompanied by an R.W.MILLER & SONS, INC. employee when looking at material stockpiles in the pit.
13. Drivers should be extra cautious and watch out for LYCON mixer trucks backing out of the shop or ready mix batch plant.
14. Drivers must immediately report safety hazards or problems to scale personnel.
15. Site seeing in the pit is prohibited.
16. Alcoholic beverages, illegal substances, and violent acts are prohibited. Violators will be prosecuted.

Before access to our facility is allowed, this site-specific information must be reviewed and understood.  When you request access for the first time each year, a signed PIT ACCESS AGREEMENT must be presented to the scale personnel, at which time they will photocopy it for our records- pursuant to M.S.H.A. Part 46.
